These forms are available to organizations that have received funding via UHC's grants. The final reports, evaluations, and cash request forms are to be filled out and delivered to the UHC offices. UHC grant numbers and signatures must appear on all forms. UHC accepts reports via mail, e-mail (scanned documents only), or fax (801-531-7869).

Allegations of waste, fraud, or mismanagement of federal funds may be reported anonymously to the National Endowment for the Humanities’ hotline. If the allegation is found to relate to issues other than mismanagement of federal funds, the NEH Office of the Inspector General (OIG) will refer the matter to the appropriate authorities.
